Proof of Residency
The Proof of Residency residency documentation is required for students entering kindergarten, all new students moving into District 31, and those students who change addresses within District 31. Returning students at each grade level will be provided with a new Residency Reaffirmation that parents will be required to review and sign to reaffirm the student’s in-district status.

 

Student Registration and Health Packets
Student Registration and Health Packets are distributed in May prior to the coming school year. The individualized grade-level health packets provide clear guidelines about the necessary information and forms for required physicals, dental exams, and eye exams.
