Financial Aid Forms
Financial assistance is available for all qualifying District 31 families for registration, activities, bus fees, and lunch. To see if you qualify for financial assistance, please do the following:
Step 1 - When completing online registration for your child, select "Financial Aid" on the "Payment" page.
Step 2 - Complete the financial aid application below.
Step 3 - Gather the required documents:
- a) If you receive SNAP benefits - provide your SNAP approval letter OR a screenshot of your online account showing your benefit period. NOTE: A copy of your Link card will not be accepted.
- b) If you do NOT receive SNAP benefits - provide three (3) most recent pay stubs for anyone on the application who is employed and your complete Federal and State income tax returns.
Step 4 - Turn in the completed financial aid application AND required documents to the District office or email them to wsteinberg@district31.net. If you have questions, please call 847-313-4416.
